The instructor you're looking for is Dennis Morton. He live in MA, but I'm not in touch with him, unfortunately. I would imagine that you could reach him through PADI, though. You can always get a copy of your card through PADI and if he still has your logbook, I'm sure they could find a way to get you in touch with him....Best of luck!
