This is a file of fatal rebreather accidents through May 24, 2015; if the accident at Key Largo was fatal, I hope not but if it was, it may show up here in the future:

I just got back from Key Largo and I was on one of the boats from the charter that pulled him out of the water. This is hearsay, but it was still a big topic of conversation.

A captain was tied up waiting on his divers to get back when he saw something that didn't look right. He pulled the diver onto his boat and he was unresponsive and not breathing, but had a pulse. He started oxygen and called for help. After arranging for another boat to pick up his divers and getting someone off another boat to help with first-aid the captain cut loose and headed in to meet the ambulance. The diver was breathing and alert by the time they made it to the dock.
