I'll keep it short Papa Bear as my info is subjective, if we're talking about the same incident we had an Instructor who has dived with us numerous times and revisit our facility dive with a girl beyond the limits we impose. The person in question was never employed by us. We can't supervise every buddy team, or diver, and don't take the police/nanny state approach to dive supervision. However once we learned that he gone beyond the safe depth limit, he was asked to leave the facility and not to dive with us again, the staff who had 1st hand knowledge submitted a report to the training agency.
