My throat and ear doctor told me to use " Otrivin 0,1% " .. it does not content any medication.

This remark rather begs the question, "Why would you want to take a substance for nasal congestion that did not 'contain' any medication?"
In any event, Otrivin 0.1% indeed does contain medication, specifically xylometazoline.
If you meant Otrivin does not contain pseudoephedrine, the active ingredient in the original Sudafed, that is correct. However, both xylometazoline and pseudoephedrine are the same class of medication, known as sympatomimetics. They both are designed to mimic the actions of adrenaline (epinephrine) and to constrict blood vessels within the nasal passages.
The only meaningful difference between Sudafed Maximum Strength Nasal Decongestant and Otrivin 0.1% is that the former is an oral/systemic product, while the latter is a nasal spray/topical preparation.
Perhaps I've missed the distinction you were attempting to convey?
