18/45 to 50/00...wow, and I thought getting 56% when I asked for 80% was bad. Hope they didn't charge you for the helium.
 
A non-Nitrox trained diver who only ever dives air is highly unlikely to own an analyzer and would therefore always assume his/her tank is filled with air.

In my locality there is no banked EAN so I usually get what I ask for, worst case I have had was having tanks filled with EAN32 instead of EAN36. I do not analyze my air tanks, except to check my analyzer occasionally, as they are filled directly from a compressor at a dive op who does not supply Nitrox
