The highest I have done was Oil Slick Leap in Bonaire. It's prob 8 feet or so from the rock to the water. Of course you can use the ladder, but it's much tougher and cooler to jump from the ledge and splash the innocent bystanders.

I did the same entry at Oil Slick. My girlfriend at the time pussed out and climbed down the ladder, BOO!

I love Bonaire. Shore diving kicks ***.
 
Its not too common, but divers can pull their groin muscles if they do a giant stride entry from over 5 feet. A few here on scuba board have talked about having to call their dives because they did that exact thing.
