I have one of these on one of my doubles:

It's a bastard to refill when it's in situ, and it gets a bit briney if you go below ~40m, but other than that it works fine & is nice when you do a long dive and/or get a bit of 'cottonmouth'
The trick there is to ensure you eliminate any air bubbles from the bag and hose when filling it and/or vent the air through the hose after filling it so the whole system is bubble free. If you have no bubbles, there is no compression and no pressure gradient to force water upstream past the mouthpiece.

The same thing applies to camel back pouches.

Yes, but it violates PADI rules if you're at a Florida Spring that advertises their water as "Gin Like".
