Probably the biggest reasons for not combining the two are political and monetary, but the ostensible reason is that traditional medicine does not recognize the validity of skeletal manipulation as a method of treating anything -- not even musculoskeletal problems.
 
Wow... I guess I haven't been to see a doctor in a long time. Do they still carry those little black bags and ride horse-drawn carriages when they make house calls?

If they're not too different, why not just fold them into an MD program as another specialty?

DOs do not want to be folded in, and they greatly value their training... it is what makes them different. I have known DOs who do go on to traditional specialist training, just like MDs, as surgeons and other specialists. The level of knowledge is the same, and they can complete residencies and become board certified just as an MD can
 
My doctor is a D.O. and since I'm terrified of doctors having someone who is willing to look for non-invasive fixes first is a blessing. He's really helped me learn to cope with doctor visits even if I don't see him as often as I should. I wouldn't trade him for the world.
