For a small, folding knife I've made a couple of sheaths using 600d PVC coated polyester with a Velcro closure. You could also add a short strap plus a side-release buckle but so far I have not found it to be necessary.. Being very insecure I also attach a strong, nylon cord with a SS clip which is attached to a D-ring. As soon as I remove it from the D-ring it gets looped around my wrist or clipped onto my backwing D-ring.

For California diving I still strap on my old BFK on my leg but this design is nice for tropical destinations because it's not very noticeable that you are carrying a knife.
