Hi all

I just joined a few days ago and was hoping someone might be able to recommend me a private doctor who will complete the PADI questionnaire/medical exam whatever it is.

I live in the UK, Tyne and Wear region (Newcastle/Sunderland) so looking for a doctor around that way. The only one the instructor knows is a Dr Crummie who only works private on Fridays and has no slots or over a month.

Unfortunately, my local GP practice doesn't do them as it lacks a piece of equipment apparently needed. And there was me thinking it would be a simple process as the only problem was having high blood pressure 12 years ago :)

This is the form PADI Medical Statement 2.01

The purpose of this Medical Questionnaire is to find out if you should be examined by your doctor before participating in recreational diver training. A positive response to a question does not necessarily disqualify you from diving. A positive response means that there is a preexisting condition that may affect your safety while diving and you must seek the advice of your physician prior to engaging in dive activities.Please answer the following questions on your past or present medical history with a YES or NO. If you are not sure, answer YES. If any of these items apply to you, we must request that you consult with a physician prior to participating in scuba diving. Your instructor will supply you with an RSTC Medical Statement and Guidelines for Recreational Scuba Diver’s Physical Examination to take to your physician.

Your personal physician should be able to determine if your condition(s) is under control with treatment or if it precludes you from diving.

Thanks for the information.

I answered the question related to hypertension/high blood pressure with YES so the instructor (rightly) insists I have the form signed off by the doctor. The problem is the receptionists at the surgery won't let me book an appointment with my own doctor because they say the doctor won't complete the form. To be honest I have no idea what is involved in a full medical, or why they can't just see that the only condition I have of anything on the list was resolved 12 years ago.
