I panicked. Yup. First pool session and in first OW dive.

Body decided that it didn't want to breathe underwater, had to slow my breathing and calm down the freak-out for a few minutes. Pool sessions went well after that, but first OW dive, same thing. Knowing that the bottom wasn't "just" 16 feet down did something and I started panicking again. Sucked through a bunch of air and pretty much halved my dive time. After I calmed myself down I was okay, but I had sucked through so much air on the surface that the dive was pretty much over as soon as I got to 40 feet. The rest of the dives went awesome though.

First mask clear in OW was at 40 feet. I had some trouble doing this in the pool (stupid nose wasn't used to not breathing) so I was a little worried, but as soon as I took care of that everything was easy peasy.
