it could of had a piece of fuzz or fiber on the seat. i had a reg do that started at 140 then in a min crept up to 200 and free flowed. took it apart inspected everything didnt see any problems put it back together and it worked fine. maybe something got in when you took the hp port off ?
 
I wouldn't worry about it too much. All warranties are chosen by looking at a bell curve of failures, and sometimes stuff does fail before the warranties. I've had regs last many years without IP creep, and others that failed in a timeline similar to yours.

I've also been guilty of diving with a primary reg that has very minor IP creep. Since you are breathing it constantly, it doesn't make a huge difference.

It's amazing that it lasted well over a year, and the first service was free. I don't think there is any other manufacturer that does first service for free and many require service done after 1 year to maintain warranties. Take the service class if you can, it's really nice to know how these things work.

When all else fails, use this as an excuse to keep buying scuba equipment. The extra regs will mean less use cycles per reg and leave you with spares to avoid missing a dive :)
