What problems have you had or know about between the bottom and the surface while deco diving?
misplaced your deco bottle?
got narced and forgot your name?
boat left to get some beer?
lift bag had a hole in it? (and you had a bunch of little lift bags?)
Inquisitive shark while hanging on the line?
jon line failure in current?
current went 180 from start of dive and now you are down stream?
buddy looks green and blue?
hole in your dry suit in 40 degree water and one hour of deco?

What you do not know can kill you, what are the problems that have solutions when you need to decompress? (please feel free to scare the untrained).:signofcross:
 
Buddy's O2 reg failed. Got bumped during the dive and depressurized. Hose wasn't fully tightened, upon repressurization the oring extruded. Not fixable. While scootering to stay with the upline we re-did the deco plan to account for sharing my O2. I passed him my bottle for his time on it. No additional issues from the reg failure beyond a few extra minutes in the (cold, 48F) water.
 
Lost the upline in bad vis. Did a free ascent from 205 fsw. Shot a bag at 135. On our shallow stops "felt" a tug towing a barge somewhere nearby. It didn't run us over ... life is good ... :D

I've seen deco bottles dropped to the bottom of the ocean.
 
Internal O-ring on 50% stage reg exploded at 50 feet (was fine at 70,60) unfixable bubbles but not enough to lose too much gas

Second stage on O2 bottle breathing wet enough to be unusable (fortunately just a practice dive)

Many many leaking SPG spindles
 
50% bottle giving more water than gas. Took face plate and diaphragm off to try and fix it but diaphragm was warped. That one was my own fault for not testing the reg while submerged before the dive.

Knob fell off O2 bottle during a dive!

In both cases just did deco on backgas and have since found that EMS shears will work on a valve stem.
 
had my o2 reg's hose oring blow with about 15-20min of o2 deco left. swapped the reg for one off the stage and it was a non issue.
 
Had a leak in drysuit, was cold as F@k. 'Luckily' my catheter had come off and when I relieved myself, it got all warm.
