I don't know the answer but remember that this was a OW student. Maybe he surfaced and didn't inflate, may he did a CESA/buoyant ascent and held his breath (which can kill you), maybe he got tangled in something, maybe he got bitten by a sea snake or stung by something causing him to panic, maybe lots of things. I hope they find out and publish the reasons too but there are many things it could have been.Wolverine:
