I just don't log dives
I unscrewed the plastic diaphragm cover from my SE2 2nd stage octo and noticed that the 3" OD nylon washer between the cover and the diaphragm is damaged. This is called a "thrust washer" in the parts list, and looks to be an anti-friction washer so the cover won't grab the diaphragm when being screwed on.

Any ideas where I can buy a replacement washer, as I don't see any for sale on VDH? Or maybe it's included with a replacement diaphragm?

you might need to find an aqualung dealer for part 821032 or call up vdh to see if they have some just not listed on the site
 
is it just cracked/broken? I have used them like that. Just put a little soap on it so the cover slides easily when you screw it down. If you inhale and get a good vacuum (with no leakage) then the case is secure and i wouldn't worry about it. Even if it were to entirely fail on a dive (which is hard to envision) you would just get wet breathing reg.

It's just cracked and I am still using it. When I examined the plastic cover I discovered a sharp ridge on the bottom where it meets the thrust washer, almost like it was designed that way. That sharp ridge distorts and folds the thrust washer over time causing it to split. I checked another SE2 2nd stage and the plastic cover is completely smooth in that area. So either a manufacturing flaw or I don't understand the design intent, but I filed off that ridge.
 
I would be a little cautious if there are any sharp edges on the damaged washer that might damage the diaphragm, which would be more expensive to replace.
 
Try your local scuba dealer. If no joy, a sheet of nylon and a circle cutter should solve the problem.
