As for free, another vote for Picasa and GIMP.

For me, personally, you can't beat photoshop's bandaid tool. Hands down the easiest and quickest way.

Lightroom's (at least lightroom 2) spot removal tool is a bit of a pain for backscatter. It leaves too many footprints. I like to thoroughly go over a picture if I am going to use it for something, so the bandaid tool is great...just click and its gone.
