Found it: Jeffrey N Cowen, Specialist in Cardiology and Hyperbaric Medicine; Frederick, MD 301-663-5790.
I've never used him, but he is the one that DAN recommended locally.
The ones in Durham that recommended are: John Jacob Freiberger (919-668-0005) and Richard Moon (919-684-6726)

Thanks for the shout-out motorref! For clarification, Drs. Freiberger and Moon are both anesthesiologists with subspecialties in undersea and hyperbaric medicine. They would refer the OP to a cardiologist in-house for a cardiology workup. Our other hyperbaric attendings are Drs. Bruce Derrick, Bret Stolp and Claude Piantadosi; all are equally capable of performing a fitness to dive examination.

Washington, D.C., and the surrounding areas (e.g., Bethesda, Alexandria, etc.) have a ton of doctors. Don't get me wrong, Duke is great. But 3.5 hours is about 3 hours too long to travel.

I realize that I'm saying all this and not giving you any recommendations. But I would call GW and Georgetown before giving up so quickly.
 
Richard Petty might be able to get from DC to Durham in 3 1/2 hours, for us mortals it's a bit longer :) We would never discourage the OP (or anyone else) from trying to obtain treatment closer to home; however, we are happy to provide care to anyone no matter the distance from our facility.

For anyone that it still following / interested, my local cardiologist (Greg Kaufman, Cardiology Associates of Fredericksburg) has signed off on my return to diving, 9 months after my bypass surgery; big thanks to Dan Nord at DAN for providing me the information / ammunition about the Bove study that helped convince him to let me dive again!
