Never. I use a traditional safe second. That's why I said "seems".
What's your take on air sharing and controlled ascent w/ ss1?
My take, is I’ve not had to do it in 54 years (no safe seconds then) so I see it as an inconvience that is unlikely to ever experience but easily done if necessary, I almost never have air in my wind except on the surface anyway, my dry suit vent automatically.

In my 51 years of diving I like to take advantage of all the modern conveniences. I probably fine tune my wind at least a couple of dozen times per dive to maintain an optimal state of buoyancy wherever I am. Honestly, I don't have enough confidence in my d400 or double hose to dispel with a nice breathing octo. I never considered carrying a safe second to be an inconvenience.

Try a D420 or G260, I see those as modern convenience.

My vote is: stick to conventional equipment. No SS1, no power inflator lever on the hip built into the BCD, etc. It'll hardly ever matter, but in an emergency, familiarity is good.

And, get a spare 2nd stage that's the same quality as your primary 2nd stage. (I use Atomic, and I don't doubt the SS1 in this regard.)

Well, the 420 turned out to be a disaster . I do have a g250, but I much prefer the breathing characteristics of the d400. The 260 is too rich for my blood for what I understand to be minor improvement over its cousin.

I ave 5 of them, they have a quirk or two but I find them superior to the older D’s in the water. The G260 is just about the best second SP has ever made but I only have one of those.

If going to use a BCI then the primary needs to be on a 40 inches hose with a swivel. In fact there is no reason could not do a 60 if really wanted but for me a 40 is good. As to venting and taking the BCI from my mouth to vent, I dive a wing and use the butt dump 96% of the time. I am not going to need to remove the BCI from my mouth to vent, so that is not a problem and a 40 inches hose gets the OOA diver out of my face. The BCI breaths good enough for who and what it is for. For deeper or more challenging diving or buddies sometimes of unknown quality I revert to a standard Hog rigged set.
