I have difficulty keeping proper trim in that my head tends to rise higher than my legs. Staying horizontal takes more effort than it should.

I have a weight-integrated BC (Sherwood Avid) that keeps the weight a little higher on my body than if I used a belt. There are two main pockets in front and two smaller trim pockets at about the same height in the rear.

It seems like the best solution would be to position some of the weight near my shoulders, but I cannot think of a good way to do this. I am considering a tank weight that I would position just above the upper (the Avid has two) BC band.

Any suggestions?

There are long skinny soft weights designed to be worn around the ankles. They also work nicely clipped around the neck/valve of a tank.

You could also try moving the tank higher up (moving the tank bands lower on the tank), and using fins that are not negative buoyant.
 
If the weight is higher on your body than before, you need to consider carefully why are you floating more head up? Sounds like the location of the bouyancy has changed, not the location of the weight.

Make sure you are not over weighted and carrying more air in the BC at depth than you really need. And depending on the bouancy traits of the tank, you may want to consider moving it up or down. If you dive an AL 80 and float head up toward the end of the dive when the tank is bouyant, you may want to move it down with respect to your body rather than up.

I am not familiar with the avid, but if it has adjustable shoulder straps you may also want to try adjusting the shoulder straps so the BC (and the bouyancy) rides lower on your body.
