Where did you get your *angle adapter*??
That one looks like the Scubapro version, but there are others available.
 
Have to admit, Pug, that I would opt for the angled adapter if I could find one in the area.

As far as I have been able to determine there are 3 classes of swivels.

1. Promate/Trident. ~$20. Bright chrome. Regular reports of failure here on Scubaboard.

2. M&J Engineering (Amron/Oxychecq) $45-$55. Matte finish. Noticeably better made than the above but an interesting failure mentioned in the thread below. Serial numbering of each unit allowed for a range recall.

2. Atomic Aquatics Universal Comfort Swivel. Available only at Atomic dealers but works with any standard second stage. A true ball and socket joint rather than hinge and swivel like the others. Comes with hose attached. Custom lengths available. The only one that should be consdered if you are looking for a swivel with no reported failures.

Uncle Pug:

UP, do you think something like this (or possibly even the 90 deg elbow) would be appropriate to use directly on a first stage? I have an Apeks ATX200 where the LP ports are angled slightly upwards. I want to route the DS inflation hose under the left arm, so I'm wondering if I were to install one of these on one of the LP ports it would create a more direct routing for the DS hose. Thoughts?

There is an angle first stage fitting but the above pictured fitting is only for the second stage end of the hose.

That said, I don't think you need to worry about adding one to the ATX200.
