Getting lost is probably the worst case scenario. You might surface and find yourself in Manhattan.

Heard a weird rumbling once while in a cave with an instructor. Kinda worried that it was not a good place to be at the time but the quake could have been a very long way away since sound travels so we'll underwater.
 
Loma Prieta and other local earthquakes. Earthquakes travel in shock waves on the surface of water. So think of swells on the surface of the water. Then think of swimming pools loosing a foot or more of water due to shockwave/swell on edge of pool and swimmers/waterpolo players near the edge of a pool being tossed out. So a diver on the surface near a boat or surface rocks or shallow reef may feel it or have similar problems as the swimmers and waterpolo player did. If out in open water then may notice the shockwave just as boats when it passes.
