lucky technicians with the presence of mind to shut it off.

I remember that picture of the club dive boat that Phi Le was on and posted the writeup of the accident on The Deco Stop.
Phi was a cave buddy of mine back when we were members of Odin Tech Divers around 2000.

Another reason for us all to consider those new AP valves for our 100% cylinders, both CCR and deco.

Talk about Solving a non-problem. Do you realise how many millions of standard valves control O2 in the medical and industrial environments? Best prevention is keep stuff clean and open valve slowly. DId I mention keep stuff clean? Of course it really means NO hydrocarbons.. At the hydro we throw out a lot of oil contaminated valves. Grease monkeys like to do the WD40 on the cylinder cap. All that and O2 fires are still very very rare.
