In many cases you need a doctors sign off only if you answer yes to a potential contraindicator to diving.

The best thing to do is to run it past your physician if he/she is dive savvy, if not consult DAN. This is about your well being , get good information.

I think it was the latest issue of Scuba Diving or Dive Training magazine that ran another artlcle of drugs and their relationship to diving.

Ive been diving for 5 years, all over the Caribbean, and Ive never been asked to provide a medical release form. Any reputable resort or dive shop will require you to sign a liability release form however, which is standard practice in the industry. They may also ask to review your logbook to verify your diving experience, which has never happened to me. Simply showing your C-card and signing the release form is the norm.
 
I've never been asked for a medical release. Given the number of times I've had to sign waivers, I'm surprised that I've only been asked for medical releases when I'm taking a class. That being said, two points, although they may be a bit off topic:

1. The latest news out of California is that an appellate court has just invalidated a waiver when it purported to release future claims for injuries that arose out of the violation of a statute. In particular a member of a health club slipped and fell in the pool area, allegedly as the result of an accumulation of algae on the pool decking. He had signed a waiver when he joined the club. However, a state statute and municipal ordinance required the pool to be kept safe and clean. Since the waiver would have negated liability for the statutory violation, the waiver was deemed void.

2. How often do people get tetanus vaccinations? As I diver, I try to get them updated every 3 years.
 
I have never been asked to provide a medical release. Shoot, I rarely have had anyone ask to see my c-card.

Good question ItsBruce. Other than tetanus, are there any other vaccinations that divers should make sure are up to date? What about for trips to the Caribbean?
 
I wonder if a flu shot (riding in planes, etc) would prevent more deaths than the tetnus shot.

I plan on having my Dr fill out the waiver form every year when I have my annual physical. Then I put 10 copies in my log book so I'll have one available whenever needed.

Is there a standard waiver form? If so where could I get a copy?
 
Hello all, this is my first posting here. I have just started taking the PADI course and have done the first module and first 3 skills in the pool. My question is on the medical. I had back surgery this past April and wanted to ask if this would limit me or keep me from diving? I do have back pain but that is something that I will have from now on though. Thanks and look forward to hearing more and learning even more.
