Dive boat etiquette: what do you do if the diver with the longest runtime is the slowest?

Seems like it. Agreeing on time-back-onto -the-boat avoids all such issues. He goes in the water late, he has to cut run-time short. Done.

Personally, I am not my brother's keeper. If they are running late and it is an issue for the next charter, it is not my issue. If they accidentally went into deco and have to hang for a while, they have to hang for a while. It is not my problem. I might say something if I see something that creates a hazard or damages the reef, but I am not the DM or the captain, so it is not my place. If it was something I felt strongly about, let the people that are paid to run the boat handle it.
The OP specifically notes run times and deco hangs. So this, to me, would not be a rec trip.
All of the boat dives I did where deco was involved were by invitation only, and the person making the invites booked the boat for the entire day.
So there was no BS about getting back for the afternoon trip. We might do a second dive, and it was up to each individual to decide if they wanted or were able to.
The person doing the invite would pick the wreck and an alternative, just in case. They'd let everyone know and give the depth and maybe points of interest.
Then each diver or team would decide their profile/plan. They'd present that to the organizer and they would dive their plan.
Since a 2nd dive was optional, there was no complaining about someone taking a little extra time to get in the water. As long as they stuck to their run time, it was known in advance if they planned on 2 hrs starting at 10 am and didn't get in until 10:30, it was known to start looking for them to surface at 12:30 instead of noon. And the delay might mean that optional 2nd dive would just not happen.
This was all made clear before anyone even got on the boat.
But that said, no way would the grill not be started at pre-agreed time. They fuss around, they get cold burgers or maybe none, and have to make do with whatever is left.

It’s in the tec forum for a reason. It’s usually one dive a day - sail for a couple hours to a wreck, put a shotline on it, jump at slack. Most skippers would ask you to limit your runtime at 2-2.5 hours.
The goal is usually to stack divers by runtime, so that people with the longest runtime jump first. Main reason is safety and comfort.
If it is DSMB up from the wreck + drift deco and you jump last and do the longest runtimes, you will be very far away from all the other divers. The boat will either need to move between you and the cluster of other DSMBs or just leave you on your own and pick up later once you surface.
If it’s a deco station - lazy shot, everyone else will be holding onto it until you start surfacing and disconnect it so that everyone can drift with the station. A bit annoying when the current picks up.

Here’s an update: we all need to catch a ferry in the evening right after the dive and @Wibble is again planning to jump last and do 2.5 hours runtime.
We are contemplating what’s an acceptable level of equipment to leave him on the deco station. So far we have a spare DSMB, fluorescent tube, soggy sandwich and some water. I suggested a rubber dinghy and a PLB but the rest of the group thinks it’s over the top :-( Someone also offered to swap fins with Wibble as better fins would help with swimming back to shore.
More details: water temp about 12 degrees Celsius, maybe 2 hours offshore in North Atlantic, weather is “grand” (slight drizzle, F2-F3).
