does the prism have 2 mushroom valves? for them both to fail to the extend of co2 build up seems very unlikely.

Everything has 2 mushroom valves. The failure of EITHER one will have the same results.
If the inhale fails, when you exhale you fill the inhale side of the loop.
If the exhaust fails, when you inhale you suck back in from the exhale side of the loop.

Could you have switched to bailout at depth and breathed it a short while to see if the weird feeling went away?
