It's a pretty darn embarrassing thing to admit...but I must anyway. 
I have a notebook with every dive listed. My log book gets updated whenever I finally get off my lazy aft-end and just do it. Significant events such as a class that requires an instructor's signature for certification always take priority. 
I'll try to do better this year.
