Hello all,
I would like to ask for medical advice for diagnosis.
On 30th of July 2017, I was diving in Bali. It was my first dive after completing OWD about a month ago and the only dive ever since. Maximum depth was 20 m and the time of dive about 40 minutes.
I had a problem with equalization of pressure in the ears, especially the left one. It took me maybe three times more time than to others, but at the end I was fine, no pain or anything, but the equalization was pretty hard (also with a little bit of pain, but not that kind of pain I have never experienced in my ears before. There was also no sudden relieve or blood or anything else.). It is also possible I did the Valsalva maneuver too forcefully.
At the bottom I was completely fine. The ascent was with group and no-one had any problem. I did feel during an ascent that I am a little dizzy and have dry mouth, but I have assumed that is only because it is my first "real" dive.
Right after I was at the top, I started to have vertigo and vomited a lot. I have ended up in the hospital where they gave me antiemetics, rehydratation and some vasodilatants and left me after two hours to go home with pills for next three days. The vertigo was gone on the next day. I remember to have tinnitus in my left ear since I have come to the hospital, I am not sure about the time before.
Since then it is two weeks and I have visited ENT specialist back home (but they do not have a lot of experience with divers over here). Audiometry showed a medium deficit (50 dB) at frequencies higher than 4 kHz on my left ear and I still do hear tinnitus and have a sensation of a pressure in the ear. Tympanometry showed about a half lower "A" on the left ear, but the second measurement at different institution the next day showed no differences between the ears. During the normal day, I do not find the deficit of hearing because it is in high frequences, but when I try to use earplugs I have a feeling of hearing the left earplug more from distance. The ENT specialist prescribed me corticosteroids, vasodilatans, calcium, antitrombotics and hyperbaric treatment (10 x 2 hours in 1,6 atm with 100% oxygen).
So far it is a fifth day of intravenous treatment and day 3 of hyperbaric treatment. I do not really want to make assumptions, because I clearly have different view when I am in the hospital (tinnitus was almost not annoying at home, here there are times when it is annoying), but maybe it is a little bit better in the morning and then gets worse, but it can be only my feeling.
Does anyone has an idea about the diagnosis or at least the next steps for the diagnosis? Any recommendations?
