Which is normal practice. Everywhere I go outside of the uk asks me to fill out one of these forms. We ask club members to fill one out each year. If a proper medical from a proper dive doctor is offered then that does. In the UK your normal doctor/general practitioner is not considered qualified.
I received a response from Sandals/Beaches in regards to the medical release question as below. I suggest printing it and taking it with if you are worried they are going to require a doctor's release, even with all "no's" on your medical form. If you want the specifics on the Sandals/Beaches Elite Diver Program, please email me at scubaone@scubaone.com and I can send it with an attachment from Sandals, that I don't know how to add here. I ran into Mr. Clarke, the watersports director for Sandals/Beaches, walking the floor at the DEMA Show last November and forgot to ask how the Elite Program was working for them.

@Duke Dive Medicine is probably the best resource here, else the good folks at DAN can probably put you in touch with the right peopleI have a question regarding Meds, that i hope some of you is able to reply to.
I have a son that like to try out scubadiving, he is on prescription meds against ADD (attention performance drug), is those kind of drugs usually ok for scubadiving ?
Offcource we are going to ask his doctor before i let him take the OW class or Tryout Scuba-class, but is there any reason to not get him exited and talk scuba with him and then he get dissapointed when the doc says no?
