Wanted SSI Dive Table Card For Binder

I know now a days most people dont use dive tables, but I am looking for an SSI dive table card. I talked to my LDS and SSI and they no longer issue them. I have looked on ebay. I know i can buy a generic card, but I am specifically looking for SSI....

does anyone happen to know where i can get one or if they have one they no longer need?

Also, by chance a nitrox for SSI?
