Sorry, I wasn't very clear in the last post. I linked a .pdf file in the above post. It illustrates a very similar story (if not identical) to the cave incident. I think this may give the details you were looking for.
 
To answer your question Citydiver, I only mentioned Ernie's heading out to sea to point out that we had gotten a little further out than usual & that my guess has always been that Ernie, faced with a fairly long swim back to the beach, had become physicaly & perhaps mentaly overwhelmed. (Ernie had been a diver (years before) on the Quincy, Mass. fire dept. dive team & had always reacted calmly to any potentialy dangerous situation that we had ever encountered) Other than his (unknown to us) heart condition, I can only guess as to what may have brought on the heart attack.
 
I'm sorry for your loss op I had a similar case I wasn't diving but out with my family boating

So here is the story was supposed to go out on the boat around noon but extenuating circumstances only got us out around 3pm we were at the boat launch at around 7:30pm I was walking to my truck to retrieve my boat when a gentleman came running up to me and asked is that your boat there I said yes expecting him to complement me on how nice it is that was not the case. He said there's a diver in trouble I asked where since I didn't see him coming in he pointed him out to me and I turned and ran to my boat gave the keys to the truck to my wife and said take the kids to the truck I will be back in five minutes I didn't even give her time to respond I was about fifty yards from the diver.and asked if he was ok I couldn't move fast due to submerged rocks he didn't respond but was flailing his arms so I asked again by making a big O over my head no clear response so I gunned it I figured if I damaged my boat on a rock the insurance would pay for it I was about 10 yards from said diver and he went face first in the water as if to take off his fins I got to within five yards didn't see any bubbles and went oh crap here we go put on my life jacket shut down the boat jumped in and rolled him on his back went to ditch his weight belt but he already ditched it and yelled for help his wife who was on shore heard me yell as well as another boat his wife who I did not know at the time was halfway about 150 yrds from me I told her to go back to shore and that she did not want to be there that's when she told me I can't he is my husband then I cursed a whole lot more the 2nd boat got to me as she was getting close so I told them to throw her a life jacket and then sent them back to the launch to give ems our exact location and then I told her to get on my boat and put it on she complied then turned around to see him not breathing and have purple lips she screamed he's dead he's dead he's dead then jumped in to try to give him mouth to mouth I stopped her asking if she was certified she said no I then told her she could do more harm then good without proper training at this point ems fire and police were coming down the driveway to the launch the fire department launched their rescue boat aND I told the wife we gotta get him close to my boat so they can see us better (I was doing everything I can too keep her from panicking and to give her something to do ) as the rescue boat came around the corner I told her to wave her arms as big as she can so they see uskeeping her busy and all as they got close we loaded his body onto the boat and I told them to go go go they left us I didn't even let her get on the boat I told them that I would take her back on my boat we climbed aboard and I took my sweet time getting back I didn't want her to see them doing chest compressions on him I heard the ambulance take off and got back to shore I had the fire department trying to tie off my boat and I got mad at them and said I need to do it after I tied off my boat and saw that the authorities had everything under control that's when I broke down he sadly passed away and it took me over 2 months to get back in the water my


It's funny how clear headed and how everything seems to slow down when your in a situation I am sorry for the long winded story and lack of punctuation please don't berate me on this
