In less than a month, I ran into yet another Poseidon regulator, whose IP was way out of whack -- and rocketed past 12 bar, within moments, of opening a valve. At least, the OPV did its job. Don't know that I had ever heard one go off; sounded like an old dieseling Datsun.

Upon disassembly, I found several tiny fragments of brass -- a couple of which were embedded in the seat itself; and clearly shows what 250 bar of air pressure can do with particulates.

As best I can determine, they may have originated from an over-torqued valve stem, whose interior appeared a bit "raw" and partially stripped under magnification . . .
