As far as I know, while Sheck was a high school teacher he was not primarily an OW instructor. I also think he wouldn't blame any environmental conditions (outside maybe a cave in) as a failure point, but rather underestimating the impact of environmental conditions, and not being properly prepared. I would say all those apply to this situation.Anyone here a fan of Sheck Exley? He would say poor viz was not the failure point of this fatality.

Exactly. Sheck differentiated between contributing factors and the actual failure point when conducting accident analysis.
