The above link points to a copy of the latest revision of my dive log inserts. I am planning on making these inserts available online free of charge shortly but first would like to post it here on the board for comments.

A note about the template: I have been revising the template over the past 3 years and only recently changed the page size from metric A4 to imperial Letter size, hence the filing holes and the margins probably are not lined up or aligned properly at all yet. The training log (at the bottom) is a template I have been using, revising, and improving for my own needs when running my courses - comments are welcome. I would basically substitute a normal dive sheet in my log with a training log sheet when I do training dives, so I would have training log inserts interspersed in between normal diving inserts throughout my binder. I have printed the training inserts on different colored paper in the past so it's easy to pick out the training sheets from in between the normal ones.

In the past I had these sheets printed on A4 paper and then got the printing shop to guillotine the pages in half for me to give me A5 sized inserts. This is something I'll still have to sort out with a printing shop here in the US to changing the size to 5.5"x8.5" (half) Letter sized paper.

Well, there it is...I'm open to constructive criticism! Let me know what you think. :coffee:
 
I like your format. Very clean. I personally like to write quite a narrative, so I appreciate the ample comment space. I particularly like your scale for visibility, I've never seen something like that before. I also like your EANx exposure calculations block, something neither of my log forms currently have. Keeps it in front of your face, nice safety item. Most everything else is standard info, but your layout is quite nice. One question.... in the temperatures block, what does the wave symbol mean?

Nice job. I can see myself using this form.

Above the "waves" you'd enter the air temperature and below the "waves" the water surface temperature when starting the dive. My dive computer also gives me a temperature at the end of the dive which I'd then enter into the appropriate box in that area. :D

Slightly off topic (or not...) but I always thought of including/creating an insert that reminds me of, and records, specific emergency skills that should be routinely practiced. The idea was to insert these between every 3 or 4 normal log pages as both a reminder of what should be practiced as well as a log of how often they were practiced.

Do you really want to log this though? Habitually practicing all skills on a regular basis should be something you do anyhow and shouldn't need to be reminded of, but I guess if you wanted to you could create a log insert for that if you felt like it. I just prefer to keep my log book for logging dives - it'd grow too much into a heavy, "busting out of its seams," handy door stop type log book if I added inserts for everything else.

I do, however, have a personal training log section with different inserts that record my own training course details; i.e. those courses where I receive training myself.

Up to you to include what you wish though; it's your log book :coffee:.

That is VERY cool! I have one (maybe stupid) question though:

I see that there is 2 profiles (PG in/PG Out) in the upper left corner, but each sheet is for a single dive. Is the first profile a carry-over from the previous dive?

Yeah I use it for repetitive dive profiles. I'd basically transfer data from the last dive profile grid on the previous page to the next when I do rep dives. :coffee:
