Two things come to mind. It's "detuned" to avoid freeflow and the resistance is a bit higher. May need an adjustment, or may not need anything if it doesn't happen underwater.

My second thought is you've got COPD. Did you ever smoke?
 
likely a sticking exhaust diaphragm. Depending on how old it is, it may be stiff or a bit stuck. Make sure to rinse through the second stage very well when you are done diving which protects that diaphragm. If it needs to be replaced, it's only a few dollars and is a DIY replacement.
