Personally I would go for the ATX50/ATX40 combo. I currently dive a ATX50/AT20 combo. All of the apeks regs I have tried breathe great. To be honest I personally can't tell much of a difference between them.
 
I always have some redundancy. I have a Vytec with transmitter. No extra pressure gauge. So when it should fail I would abort the dive.
I carry two lights (main and backup ; where I dive it is usually murky, dark, and cold: the Netherlands) and a bottom timer (hyper aqualand)

But, when something really goes wrong... abort the dive gracefully.

I never did some cave diving. Then I would add the extra pressure gauge.

I would consider getting two ATX50's second stages but with the slightly cheaper DS4 first stage instead of the DST first stage that has the useless swivel turret and funky 1/2" LP port. You can spend more money but you can't get a better value. YMMV.
