Not me. I avoid this problem by keep my nuclear-cooling-tower snorkel far far away from me. But I have been entertained by a person who did.

Too many times, and sometimes with it around my neck.

Ever descend without your tank on [tightly]?

Without the gas turned on? No, but I've jumped in with the hoses charged but the valve off. Without it tightly secured? No, but it's worked its way loose once or twice. No big deal, cause I can reach the cam strap and buckle with the BC on, so I can fix it without assistance and without taking the rig off.
