My team is looking at updating our divers pre & post dive medicals. Currently we have dive medics on the team who check each diver prior to their dive and then once they have completed their dives. Info obtained is blood pressure, O2 saturation, body temperature, pre-existing injuries to include minor cuts and abrasions, simple questions like how are you feeling today, etc. We have a very basic form the medics complete, and we maintain these forms. We are looking at possibly creating an EXCEL document that the medic can input the diver's info so that we no longer need to keep up with paper forms.

What are other teams using? If you have a paper form are you willing to share? If you are using a computer program or EXCEL are you willing to share?
