Recently there has been some discussion about PADI and it's QA procedures. Well, I just received their questionnaire and thought I would share it with anyone who is interested.
It is a multiple choice with 'Yes' 'No' and 'Unsure/NA'
as follows:

Q1. Does the Instructor's name on your certification card match the name of the person who conducted your final open water training dive(s)?
(If you did not have your card yet, please check with your PADI Instructor, Dive Center or Resort.)
Q2. Did you complete your course as a referral student?
(That is, did you begin your training at one location with one instructor, and finish it at another location with a different instructor?)
Q3. Did you complete a Medical Questionnaire prior to any in-water training?
Q4. Did you use the PADI Open Water Diver Manual (GO DIVE) and video,or Multimedia CD-ROM for your course?
Q5. Was your academic knowledge assessed by a written OR oral quizzes and a final exam?
Q6. Did you learn how to use the Recreation Dive Planner (dive table) Wheel or Table version?
Q7. Did you initially learn and practice water skills in a pool or confined water?
Q8. At some point before certification, did you complete a 200 metre/yard continuous surface swim OR a 300 metre/yard swim with Mask, snorkel and fins?
Q9. During your pool/confined water skill development session, did you practice:
Regulator Recovery and Clearing?
No Mask Breathing?
Out of air/low air emergency ascents?
Buoyancy control?(fin pivot and hovering)
Disconnecting the low pressure inflator hose?
Equalization techniques (of pressure in ears, mask)?

Q15. Did a PADI Instructor personally evaluate your underwater diving skills during the open water training dives?
Q16. During your open water training dives, did you perform the following skills:
Removal and replacement of mask underwater?
Compass navigation?
Buoyancy control?
Removal and replacement of weight system?
Alternate air source (such as an octopus) ascent?
Controlled Emergency Swimming Ascent (CESA)?
(This skill involves a simulated out of air situation where you swim to the surface, regulator and weights in place, making a continuous exhalation sound.)
Q17. If you performed a controlled emergency swimming ascent, did you instructor:
Ascend with you to the surface?
Maintain physical contact with you?
Maintain contact with an ascent/control line?
