For IDC Crew Pack it's a blue one. You may ask the guy who didn't use it why the hologram is missing.

Only reason it worked for me is that I had a clear "chain of custody" of my Crew Pack (dive shop order from PADI, PADI ship to shop, shop gave to me) to prove that I had actually purchased the crew pack and not received it used from someone else. In lieu of the hologram I had to include copies of the order, the packing slip, etc with my application. OP wouldn't be able to provide that.
