OMS Tank Valve issue

I heard about this from my local dive shop last week and saw this note on a local dive club site -- please take note.

"It has been report that a few of our local dive shops have reported that OMS Valves that are marked "30-00" on the face of the valves, have a defective end cap that could unexpectedly let go, possibly causing serious injury. OMS is replacing the caps, but they are not publicizing it.( It is not on their web site.) Please put out the note that if anyone has an OMS valve with these markings, they should immediately drain it and contact either OMS or their local dive shop."

Apparently 2 shops here in the Boston area have experienced this problem first hand -- one on a tank with less than 1,000 psi (fortunately, they were closed and no one was in the shop because the cap shot through a wall).
