It is the standard medical release form used by the RSTC, the WRSTC, major agencies, etc. It can be found here: (NOTE: an older version of the form asked questions about COVID. It is no longer valid.)

And in the U.K. we don’t accept any GP signing this form, it must be a member of the UKDMC. Age is not a trigger to contact a diver doctor.
