Jag - I haven't been to Narita with my card yet - however according to the info book that goes with it a Gold card gets you into all Amex lounges worldwide. I'm not sure if Amex has it's own lounge in Narita - it might be combined with the JAL lounge, I believe it is in Fukuoka for instance.
