I always use the Banco del Norte ATM

When I was in Puerto Vallarta there was a bunch of Banorte (aka Banco Mercatil del Norte, SA) ATM locations throughout the city. That's what I used and never had a problem. The one's I used on Cozumel are called Banamex (Banco National de Mexico, SA). It sounds like both of these are fairly safe, except I missed the step down coming out of one in Vallarta :| Some hotels (resorts) have them in the lobby but in Vallarta I kept finding machines that only took VISA and we only had MasterCard (debit cards). I never had that problem on Cozumel.

Besides, you need to pay in cash at MEGA or Chedraui so you can give your coins to the person bagging your purchases :) I generally tip about 10 pesos or whatever I got from the cashier. I've also heard a rumor that there is an English line at some MEGA stores but I've never looked for one. Also, those expensive-looking shops downstairs often have some very good prices, at least they did in Vallarta. No bartering though. Both stores are like mini-WalMarts and have a lot of merchandise and some good prices. Vanilla is about $4 a pint (half litre?) instead of $10-$15 US but the bottle is not as thick.

My credit union would have as well, except that I wasn't at home to receive it. They wouldn't send it to me on Cozumel, but I had a backup card on another account, so no great catastrophe. I was really careful with that one!
 
We always use our credit card at Mega and Chedraui - they convert to pesos, so you get the current exchange rate. And our Barclaycard charges zero foreign transaction fees.

A word of caution: shortly after a trip to Cozumel my credit card was compromised. Using it at Mega or Chedraui or some other place where it does not leave your sight is probably safe but I would never use it at a restaurant etc. I can't prove it happened in Mexico but I have my suspensions.
