If anyone is looking for an electronic version (excel) that updates times for you, I have one located on my website for download here. Just go to the bottom of the page... I found this on the web a year or so ago and made some subtle changes to it to suit my needs.
Warning: Do not fill out the second page. Or, if you do, don't let your non-diving signifcant other see it...:no :popcorn:
 
Well.... here it is. It's metric so some of you will have to take teh time to convert it to imperial units. Nevertheless it's (in my opinion) a good enough framework to build on if you're keen on making a logbook in Excel.

I took one of the Day Planner type sheets, ran it off on a copier, magnified it so that it would fit on an A4 sized page, then used a three-ring punch to put holes in it. I put it in a cheap three-ring binder. I can run off as many as I need, whenever I need them.
