I am looking for a woman that was my original dive instructor in Maui in 1987. Her name was "Chucky" and she was about 30 years old at the time, give or take. She was in the Kanapali area at the time. I'm told that she and her husband (maybe "Reggie") owned a small dive shop in Kehei sometime after 1987, but I don't know any other details. I am not sure of the name of the dive shop that she was with when I was certified.

If anybody ever heard of Chucky or can help me find her real name or anything about her, I'd appreciate it. I'm told that she moved to the mainland some time back.

Here's the deal: I recently misplaced my PADI card. PADI says they cannot find me in the data base. They tell me that they might be able to help me if I can identify my instructor.

Please help, if you ever heard of Chucky.
 
Jim, If the card was indeed issued ant PADI shop or pro can help you. The more info you have the better. BUT it needs to be exact info. Do you remember exactly how your name was listed on the card? For instance if the card was issued to JAMES D SMITH then an inquiry for James D. Smith will not show up. Punctuation and capitalization are important. As a DM I can check the database but I would need you to PM me your details such as name exactly as it appeared on the card, birthdate, address at the time the card was issued, date it was issued, and if you could possibly find it the name of the shop and instructor. Most important for me to look it up is the name part and birthdate for online lookup. The other info would only be needed if I had to call them and try to find it with as much info as possible. PM me or go to any PADI shop. You could then order a replacement card for 35 bucks.
 
Many times the birthdate is switched which can also screw things up. PADI wants the international notation: day/month/year. Many American shops put it in accidently as month/day/year. If your birth day is May 10th check your name under 5/10 AND 10/5. That mistake happens all the time.
 
You might try posting this or asking it be moved to the Hawaii O'hana forum. Twenty years is a long time, though.
 
They aren't obliged to keep records for anything like that long either.
 
I spent maybe 45 minutes on the phone with PADI and the extremely nice lady there tried searching everything. I'll drop by my dive shop to see if we can search more. If I could find Chuckie, if sure would speed things up.

Wouldn't happen to have an old logbook where she signed a dive etc with an instructor number?
