Doff and don with surfacing between used to be a skill. I had to demonstrate a doff and don for my SDI Solo card not that long ago. We did one on the bottom, I left my rig on the bottom, but I did not kneel on the bottom. Then we did another in open water while swimming, doff, swim with the rig in hand and then don it. But, we were in 80 feet so I did not surface and the dive back down. When I did my AI course we did a doff-ascend-surface dive-don and we were in 20 feet or so. But, yes, used to be a skill. But I am not speculating on the instructor or cause of death.
