Fail's a strong word. I prefer "didn't pass on first go". Example, I had a guy that recently didn't pass an intro to cave course. I outlined exactly what I expected from him, and he's practicing within the limits of his current training. We're getting together in a month to see if he's ready or not.
I've also had times where I've stopped the class mid-way through, articulated what students needed to work on, then we resumed a few months later and they passed. Would these count as a fail because I stopped the class mid-way through?
Generally speaking, 1 in 4 don't pass my courses on the first go. In some cases it's something minor and we can fix it pretty quickly and they just needed that kick in the butt to realize that yes I was serious, in other cases they need significantly more practice. For the most part, those that have put the effort and worked on the things I asked them to have been successful.
