So far it has been when it explodes. So I have had to replace one hose in 300+ dives.

I check them for wear and tear though, and if I noticed a problem I would change it then. I have a mixture of Miflex and normal hoses. The price difference is not substantial locally to me.
 
How often are you guys replacing your hoses? At first signs of wear or after a specified number of dives, etc? Just curious

I replace mine every three months, but the rubber in the hoses takes forever to decompose, so I recycle them. Problem is as far as I know, there's only one recycling center for used hoses in the Midwest. Luckily for me it's in St Louis. As a personal favor to you, I'll let you send me your 3 month old hoses, and I will recycle them for you at no charge.
