I'll eat the egg I have on my face... I think I'm back to looking for a DIOL (assuming I understand correctly that the oral inflator points towards my lips, not towards my buddies.
Nope... the DIOL is the one in Mo2vation's attachment.

But really... I wouldn't think it would be comfortable to have a diagonal inflator pointing in a way that would require you to shove the hose against your cheek... to me, the DIOL is okay, and the only thing wrong with the OBOD is that the inflator is on the wrong side. Still, I twist mine around so the inflate button is on the inside, and I can still get the mouthpiece in my mouth if needed.
 
Ok, let's go back to the drawing board. Maybe I'm doing something wrong?

My problem is that when I bring the inflator up to my mouth the mouth piece points over my right shoulder. I have to turn my head well to the left and lean slightly to the right to get the mouthpiece to line up with my lips and seal.

Maybe that's just the way it is?

On my other BCD's the mouth piece is at a 90 degree angle to the hose and the hoses are a bit longer. I have no discomfort with them.

Here's what I've gotten for inflators over the past five or so years...

Left to right from oldest to newest.

Left one I have on my 18P and my 45E wings, 1998 and 1999 respectively. Both round hoses, offset attachment to wing and recycling logo livery (thank you swmn for the word).

Middle one is circa 2001, 70E wing, flattened hose, center attachment to wing and blue oval, curved lettering livery.

Right one is circa 2002, 36P wing, flattened hose, offset attachment to wing and chopped up H livery.
