well it isn't typed but I used a permanent fine tip marker and put all the formulas on the last page of my halcyon wet notes. if you make a mistake or want to update/change anything some rubbing alcohol takes the marker right off. seems to work fine thus far.
on the other side of the last page I put my name, contact phone numbers, emergency contact numbers, address, DAN member number and the DAN emergency number, medical information like medication allergies and blood type, and several cert numbers like my OW, AOW, Nitrox, and the like. its a good reference should I need any cert numbers for something and if something bad should happen everyone knows where to find important information.
